(We need this to create your 1099 at year-end.)

Once we receive your W-9 and the form above, we’ll add you to our payroll system.
Square Payroll will then send you a secure link to enter your bank information for direct deposit.
You’ll be paid via direct deposit (unless we’ve agreed otherwise).
At year-end, you’ll automatically receive your 1099-NEC from us for tax purposes.

Your information is used only for contractor payments and required tax reporting. Banking details are never collected here - they are handled directly through Square’s secure portal.
